Your Role as a Clinical Nurse Case Manager

As a Clinical Nurse Case Manager, you’ll work directly with individuals managing chronic illnesses, recovering from acute episodes, or navigating the challenges of aging and long-term care. You’ll serve as a trusted advocate, resource, and guide — helping clients and families make informed decisions, access essential services, and follow through on complex care plans.

This position blends the science of nursing with the art of communication, advocacy, and coordination. You’ll act as the central point of contact between the client, their providers, and their support system.

Core Responsibilities

  • Conduct comprehensive initial assessments (remotely or in-home) to identify physical, emotional, and social needs
  • Develop, implement, and monitor individualized care plans
  • Serve as a liaison between medical teams, specialists, insurance providers, and family caregivers
  • Support clients during transitions of care (hospital discharge, rehab, etc.)
  • Perform medication reviews and educate clients on adherence, risks, and cost-saving options
  • Assist with advance care planning and legal/medical directives
  • Recommend and coordinate home care services, community resources, and long-term care options
  • Attend virtual or in-person provider visits as a support person or advocate
  • Maintain thorough documentation using EHR systems
  • Participate in peer case reviews and optional team training sessions

What We’re Looking For

We seek registered nurses with strong clinical judgment, communication skills, and a passion for helping people make confident healthcare decisions.

Minimum Qualifications:

  • Must reside in California and hold an active California RN license (required)
  • 3+ years of direct clinical experience (case management, acute care, hospice, or home health preferred)
  • Bachelor of Science in Nursing (BSN)
  • Reliable transportation for occasional in-person client visits
  • Proficient in using technology: smartphones, video conferencing, EHRs, and email

Preferred Qualifications:

  • Certified Case Manager (CCM) or willingness to obtain within 12 months
  • Prior experience in elder care, chronic disease management, or psychosocial support
  • Bilingual language skills (desired)
