Sage Path Health is a DBA of Master Quest, Inc. We provide case management and patient advocacy services across California. As a healthcare service provider, we are required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to maintain the privacy and security of your Protected Health Information (PHI).
